How to apply
Medical Documentation
Submit your province’s Permanent Disability Application to RRU’s Accessibility Services office for completion of the school section.
- BC residents complete the Appendix 8 application form.
- Alberta residents complete the Schedule 4 form.
- Ontario residents complete the Disability Verification form and the BSWD form.
If your medical practitioner has already completed RRU’s Permanent Disability Accommodation Request form, or if you have other detailed medical documentation issued within the past 5 years, there’s no need to complete the medical section of the province’s form. Fill out just the Student Section of the provincial form and submit that to RRU’s Accessibility Services.
Equipment and Services Details
For adaptive equipment, obtain two quotes for each item and submit these to Accessibility Services. Screenshots of the product, product details, vender, and price are fine.
For services, submit to us:

- A quote from the service provider that lists the hourly rate, number of hours per week and number of weeks of service (align weeks to your student loan study period), and their resume.
Accessibility Services will complete the school section on the form, provide the proof of enrollment, and return everything to you for upload to your dashboard.
